you or your company. You have a payment due that is currently on hold,
which we will release to you as soon as you complete and send back your
W-9 form.

We have created instructions on how to upload your W-9 form here:

Please fill it in with the correct information and either upload it as
instructed above, or alternatively return it to my attention by email to:

I have to say that, in the times we are in, where affiliate programs seem to close up shop without notifying anyone, it's GREAT to know that there are still some companies out there that actually care about the webmasters that are promoting them! I have to admit that I've barely been sending any of their 3 programs traffic of late due to me just getting "back in the game" the last few months. They didn't have to do this. They didn't have to let me know that I neglected to fill out my W9.

So I'd just like to publically say THANK YOU to WebMediaProz - your company is top notch in my book!!
